Description Of Service: Bridge Youth & Family Program has been designed to help young people aged between 12-25 and their families address barriers or issues relating to drug and alcohol use.
This program is about active mutual participation by both program staff and clients. Bridge Youth and Family promote and facilitate a healthy lifestyle program that operates under a case management model. Our goal is to help young people and their families deal with issues such as:
• Drug and alcohol education/harm minimisation
• Drug and alcohol use
• Living skills
• Self awareness
• Self Esteem
Once a client is commenced in the program they are encouraged to actively engages in a period of intensive support one day a week for the first three weeks. They are then encouraged to participate in ongoing case management on a fortnightly basis. Their involvement in the program will also include structured group sessions and external youth activities. |
