Description Of Service: A strength based intervention model that allows flexible entry and exit points with continued support & advocacy for vulnerable young people 12-25 years.
Supported crisis accommodation,(8 beds); medium term accommodation,(5 beds); transitional accommodation, (3 beds); semi-independent supported accommodation, case management, early intervention, post crisis and engagement in community awareness on homelessness prevention.
Outreach support, for clients aged 16-25 who reside in their own accommodation or who have moved through intensive intervention to independence, to maintain their accommodation long term. This may be achieved by providing assistance to obtain/maintain Community Housing and support in promoting independent living and problem solving skills. |
